COVID-19 Emergency Rental Assistance Program Application Checklist
Listed below are the following documents you will need to bring with you for your application. Please keep in mind that any missing documentation will result in your application not being processed and it will be returned to you for your completion.
- Government issued ID, U.S. or foreign passport, Social Security Card, Green Card, U.S. Visa, Refugee Resettlement ID, Work Permit, Military ID, or Matrícula consular
- Proof of your address (this can be a utility bill, or a letter from a financial or government institution (Letters from a bank, SNAP, HUD, schools, etc.), a current valid Driver’s License or state issued ID card, or a current and signed Lease Agreement.
- Proof of income for all people over the age of 18 in the household.
Proof of income can be verified using:

- 2020 or 2021 tax return
- 2020 or 2021 W2s
- Last 60 days of paystubs (with gross amount)
- Social Security documents with monthly amount (including monies received for minor dependents living in the household)
- Unemployment documentation with weekly amount
- Child support documents with amounts
- Zero income Form (Form can only be used if the individual has $0 income for the last 60 days)
- Landlord Attestation Form fully completed by landlord.
- Lease Agreement (Minimum Required: 1st Page: containing rental property address, name/s of tenant/s, name of Landlord, length of lease term, rental amounts; and signature page.
- Proof of housing instability, like eviction or 10 day notice, past due rent or utilities or loss of substantial income.
- Utility Bills- provide a bill or account number for any utility at the residence (If you are requesting assistance with utilities).
